Lake County General Health District Environmental Health Community Survey

The Lake County Health District and the Environmental Health Assessment Team are performing a community wide Environmental Health Survey in an effort to develop an action plan to address the most critical environmental health issues facing our community. We need your help to understand the issues and needs in Lake County!

Environmental Health is defined as the prevention or control of disease, injury, and disability related to the interactions between people and their environment.

Responses from the survey will help us to identify environmental health concerns, take action on the problems, and improve environmental health in Lake County.

Please take a moment to complete the survey as your opinions and feedback are extremely important.
