A public health emergency is any event or situation demanding immediate action on the part of the Lake County General Health District in order to prevent disease and injury or maintain public health within Lake County. Emergencies can be natural or man-made. Public health emergencies that have occurred in Lake County include flooding, derailment of a train carrying hazardous materials and H1N1 influenza.

What is the NAPH Form?

NAPH stands for “Name, Address and Personal History.” The NAPH Form is the form that every individual in Lake County will be required to fill out and turn in if the public health disaster requires mass vaccination or mass prophylaxis of the entire county.

Forms will be available at the site of dispensing, however, the line will move much faster if individuals have their own form filled out prior to arriving.  (NAPH Form)
