Great Gains
Target Population: Birth to kindergarten
Great Gains extended service coordination is designed to assist high risk families of children birth to five with kindergarten readiness. The Great Gains home visitor helps the family reinforce skill building at home, navigate the system and engage in services available to children with unique needs.
Key outcomes include:
- Home visitation services for case management and parenting education
- Helping families increase their child's kindergarten readiness skills
- Help families learn to advocate for their child
- Linking and referring families to community services
Participation in the Ohio Commission on Fatherhood, in collaboration with:
Lake County Commissioners, Lake County Juvenile Court - Judge Karen Lawson, Lake County Department of Jobs and Family Services and Father's House Ministries to mobilize the engagement and support of Fathers in Lake County.
