To submit a nuisance complaint to the Environmental Health Division, please fill
in the form below completely. Incomplete information or addresses may prevent the
complaint from being investigated. You will be contacted once the complaint has
been investigated and/or resolved.
Anonymous complaints are accepted only under certain circumstances. Please phone
(440) 350-2543 during normal business hours or follow the directions below for mailing
a complaint.
For complaints regarding the interior conditions of a house or apartment, the name
and original signature of the person making the compliant is required. Mail the
complaint form to the Lake County General Health District, 33 Mill Street, Painesville,
OH 44077.
