Clinical Services consists of Registered Nurses and Clerical Specialists. Public Health Nursing is the practice of promoting and protecting the health of populations using knowledge from nursing, social, and public health sciences. Activities are selected based on primary prevention priorities. Using available resources the nurse strives to assure the best overall improvement in the health of the population. This includes collaboration with a variety of other professions, populations, organizations, and other stakeholder groups to promote and protect the health of the people.
This program provides appropriate vaccines to the adult population for vaccine-preventable communicable diseases. The vaccine fees are reviewed annually and established by the Board of Health.
BCMH (Bureau for Children with Medical Handicaps)
Target Population: Families of children birth to 20 years of age
BCMH, the Bureau for Children with Medical Handicaps, is a health care program in the Ohio Department of Health. BCMH links families of children with special health care needs to a network of quality providers and helps families obtain payment for the services their child needs. Special health care needs may be caused by a condition a child is born with or develops later.
Children who are eligible for the program must be under the age of 21, be a permanent resident of the state of Ohio, be under the care of a BCMH-approved physician, and have a chronic medical condition. BCMH has three programs to help children with special health care needs: the Diagnostic Program, Treatment Program, and Service Coordination Program. Financial eligibility is required for the Treatment program only.
Public Health Nurses at local health departments can help families learn about BCMH and assist with the application process.
Clients may be eligible to receive nutrition services in their home from a registered and licensed Dietician.

Childhood Immunizations
Target Population: Children 2 months to 18 years of age
This program provides protection against vaccine-preventable childhood diseases through the administration of the appropriate vaccines to children, as well as, providing parental education. Ohio Department of Health provides the vaccine for uninsured or underinsured children.

This program identifies and controls communicable disease outbreaks in Lake County in order to protect the public from illness, disability, and/or death. The Communicable Disease Team maintains a system of surveillance, analysis, recording, and forwarding reports of communicable disease to the Ohio Department of Health with an emphasis on outbreak recognition, investigation, containment, and education.

Public Health Nursing offers CPR (Cardiopulmonary Resuscitation) certification classes, Communicable Disease Control, Physicians' Office Immunization Education, and Child Care Provider Education Classes.

Genetic Newborn Screening
Target Population: Newborn
This is a mandatory screening of all newborns in accordance with Ohio Administrative Code 3701.45. The Health District will provide this screening if there is a home birth with no physician or midwife in attendance or when a physician is unable to locate a baby who needs a second test or retest.

This is a service that Clinical Services provides to perform initial health screenings as needed for placement of foster/custody children through the Department of Job and Family Services.

Hepatitis C Antibody Testing

This monthly clinic provides high risk individuals the opportunity to be screened for the Hepatitis C antibody at no charge. Adults are seen by appointment.

Lead Screening
Target Population: Children 12 months to 6 years of age
Information on lead lesting, children at risk, and side effects of lead poisoning is offered routinely to clients in the Well Child Clinic and Lead Clinic. Information on proper paint removal, water testing, etc. is given to clients upon request. As appropriate, children with elevated leads are followed with additional screenings and/or referred to physicians and Health District Environmental Health according to Centers for Disease Control and Prevention standards.
All households with children with elevated lead levels receive a home visit by a public health nurse for in-depth screenings and education. All children with elevated lead levels are referred to the Help Me Grow Program as appropriate.
There is no fee involved for the testing.

Newborn Home Visit Program
Target Population: First time parents, teen-parents, and their newborn infants
Public Health Nurses make a one-time home visit in the first few weeks after the baby is born to provide an assessment of mother and child, answer questions, link families to services and resources within our community, and provide information to keep babies healthy and safe. This newborn/maternal home visit is free of charge.

Rabies Vaccine
Target Population: Residents of Lake County who had or may have a
potential exposure to Rabies
Communicable Disease Nurses are responsible for providing information on how to obtain Rabies vaccine for use in post-exposure therapy. Technical information on Rabies disease and Rabies vaccine is provided to local physicians upon request. An information packet about Rabies and
Rabies vaccine is available to clients/families in need of post-exposure vaccine.

Seasonal Influenza

Annual vaccination against influenza is recommended for anyone who wants to reduce the risk of becoming ill with influenza or transmitting it to others.

Tuberculosis Control Program
Target Population: Community at large with focus on high-risk populations such as: the elderly, foreign-born, incarcerated, homeless, and other long-term care residents
Communicable Disease Nurses receive reports of suspect and confirmed cases of active tuberculosis (TB) for contact identification and follow-up. Prevention efforts include medication therapy for positive reactors and cases and providing TB Testing Clinics and client education.

Well Child Clinic
Target Population: Underinsured children 0-21 years of age
This program provides comprehensive well child health and developmental screenings and referrals. Screenings are age appropriate and completed by a team consisting of a physician, several nurses, a social worker, and a dietitian. Sports and work physicals, pre-school and school physicals, and well baby checkups are completed. The Well Child Clinic remains partially funded by the Children and Families Health Services Grant.

ACHIEVE Wellness Lake County, which stands for Action Communities for Health, Innovation, and Environmental Change, is a county-wide partnership that consists of city and county health officials, city and county government, parks and recreation departments, the Lake County YMCA, and representatives from local businesses, community agencies, and schools. This collaborative partnership helps to implement policy and environmental change in the areas of physical activity and nutrition.

B Fit 4 Life Corporate Challenge

The B Fit 4 Life Corporate Challenge is a five month initiative ( January
through May) among local businesses to increase their level of physical activity and overall fitness. Each month includes a main fitness event and alternative activities. Points are accumulated based on the team's participation level. The goal of each team is to obtain the maximum amount of participation from the entire team. Teams can have a minimum of five to ten employees or a maximum of 20. Each team member needs to complete 3 of the 4 monthly events; 1 of which is pedometer walking. The idea behind B Fit 4 Life is to encourage employees to be active all year long.

Car Safety Seat and Booster Seat Program
Target Population: Low income families and children
This program provides child safety seats and booster seats to low-income and economically disadvantaged families. To be eligible for a reduced cost child safety seat or booster seat, the following requirements must be met:
- Child is no less than 5 pounds.
- Parent/guardian agrees to pay a cost of $30.00 for a convertible or a high back booster seat or $5.00 for a backless booster seat.
- Parent/guardian has a vehicle with working safety belts for transportation of the child and brings this vehicle to the class.
- Family is determined to be at 185% or less of the federal poverty income guidelines.
- To receive a convertible or high back booster seat, the parent/guardian must attend a one hour educational class on the proper use and installation of a child safety seat. Backless boosters are available on a walk-in basis.
- Parent/guardian agrees to the proper use of the child safety seat each time the child rides in the vehicle.
- Parent/guardian agrees to the proper use of their safety belts every time they ride in the vehicle.

A public health emergency is any event or situation needing immediate action from Lake County General Health District (LCGHD) to prevent disease and injury or maintain public health within Lake County. Emergencies can be natural or man-made. Examples of past Lake County public health emergencies include flooding, derailment of a train carrying hazardous materials, and H1N1 influenza.

Through education, awareness, and practice, we work to engage the community in healthy lifestyles that can be maintained through all stages of life.
The Lake County Childhood Obesity Prevention Initiative provides school, community, and family based education programs with a focus on making healthy diet, lifestyle, and environmental changes. The Healthy Families Resource Center offers school based support to children identified as having potential body weight concerns. The center provides office based counseling to the family, not just the child.

Target Population: Volunteer medical professionals
This program provides medical professionals with the opportunity to volunteer in the area of emergency preparedness and response. Volunteers may work and/or live in Lake County and must hold a medical license/certification or work in a medical capacity. The goal of this program is to better prepare our county in the event of an emergency.
- New volunteers must meet with the Lake County Medical Reserve Corps (MRC) Coordinator to complete volunteer information forms and be properly introduced to the program.
- New volunteers must register with the state of Ohio on the Ohio Responds database as a volunteer with the Lake County MRC unit.
- Volunteers must complete four hours of approved training every three years.
- A background check will be conducted on every volunteer.
- An ID badge will be created for every volunteer.
- Volunteers are asked to participate in quarterly Ohio Responds test activation alerts. Volunteer MRC meetings are scheduled as needed and there will not be more than two per year.

Help Me Grow
Target Population: Pregnancy to preschool
Research shows that during prenatal to five years of age there is a critical window of opportunity for rapid growth of the brain, language, motor, cognitive, adaptive, and social-emotional skill development that lasts a lifetime. This window of opportunity is extremely important for children who require extra support to reach their full developmental potential and for children at risk of not meeting their developmental potential.
Help Me Grow supports the well being of pregnant women and young children through home based specialized service, prenatal to three years of age parenting education, community resource linkages, and public awareness. A special emphasis is placed on early prevention and intervention and support to first time/teen mothers to ensure that all young children in our community are offered the opportunity to reach their full developmental potential during this critical period in a child’s life.
Families are offered:
- Full developmental screenings that include vision, hearing, and nutrition
- Prenatal education for expecting parents
- Home visitation services for case management and parenting education
- Linking and referrals to community services
- Full comprehensive developmental evaluations for children with medical diagnosis or suspected developmental delays

VITAL STATISTICS
Although Ohio records of births and deaths are filed within the registration district in which the event occurred, birth certificates are available in Lake County regardless of where the birth occurred. No matter where someone is/was born in Ohio, the Health District Vital Statistics department can issue that birth certificate. Currently, death certificates are filed and issued only for Lake County deaths. You may stop in at the office of the Lake County General Health District and obtain a certified birth or death certificate for $21.50 per certificate. Cash, checks, and credit cards (except for American Express) are accepted.
For genealogical purposes, computerized birth and death indexes may be searched between the hours of
10:00 a.m. and 2:00 p.m. Monday through Friday; no appointment is necessary. If there are other people waiting to view the indexes, time on the computer will be limited to one-half hour per visit. A place to work is provided in the Administration area where the guest computer with index files is located.

WIC (WOMEN, INFANTS and CHILDREN) PROGRAM
Target Population: Lake County women who are pregnant, breastfeeding
and/or postpartum, and children 0-5 years of age
SERVICES: WIC provides individualized nutrition education and counseling and breastfeeding education and support. WIC is a supplemental nutrition program for women, infants, and children up to the age of five. Foods are individually prescribed from
registered and licensed dietitians to ensure adequate and appropriate nutrition. WIC offers
nutritious foods to promote infant, early childhood, prenatal, and postpartum nutrition.
Some other services offered include a peer helper program promoting breastfeeding and
support, and breastfeeding consultations with certified lactation counselors. WIC acts as
a resource for several diverse community services including: Medicaid, Healthy Start,
Child and Family Health Services, BCMH, Well Care, and several others.
