CLINICAL SERVICES

 

Clinical Services consists of Registered Nurses and Clerical Specialists.  Public Health Nursing is the practice of promoting and protecting the health of populations using knowledge from nursing, social, and public health sciences.  Activities are selected based on primary prevention priorities.  Using available resources the nurse strives to assure the best overall improvement in the health of the population.  This includes collaboration with a variety of other professions, populations, organizations, and other stakeholder groups to promote and protect the health of the people.

Adult Immunizations

Target Population: Adults 18 years of age and older

 

This program provides appropriate vaccines to the adult population for vaccine-preventable communicable diseases.  The vaccine fees are reviewed annually and established by the Board of Health.

BCMH (Bureau for Children with Medical Handicaps)

Target Population:  Families of children birth to 20 years of age

 

BCMH, the Bureau for Children with Medical Handicaps, is a health care program in the Ohio Department of Health.  BCMH links families of children with special health care needs to a network of quality providers and helps families obtain payment for the services their child needs.  Special health care needs may be caused by a condition a child is born with or develops later.

 

Children who are eligible for the program must be under the age of 21, be a permanent resident of the state of Ohio, be under the care of a BCMH-approved physician, and have a chronic medical condition.  BCMH has three programs to help children with special health care needs:  the Diagnostic Program, Treatment Program, and Service Coordination Program.  Financial eligibility is required for the Treatment program only.

 

Public Health Nurses at local health departments can help families learn about BCMH and assist with the application process.

 

Clients may be eligible to receive nutrition services in their home from a registered and licensed Dietician.

Childhood Immunizations

Target Population:  Children 2 months to 18 years of age

 

This program provides protection against vaccine-preventable childhood diseases through the administration of the appropriate vaccines to children, as well as, providing parental education.  Ohio Department of Health provides the vaccine for uninsured or underinsured children.

Communicable Disease

Target Population:  Community at large

 

This program identifies and controls communicable disease outbreaks in Lake County in order to protect the public from illness, disability, and/or death.  The Communicable Disease Team maintains a system of surveillance, analysis, recording, and forwarding reports of communicable disease to the Ohio Department of Health with an emphasis on outbreak recognition, investigation, containment, and education.

Community Education

Target Population:  Community at large

Public Health Nursing offers CPR (Cardiopulmonary Resuscitation) certification classes, Communicable Disease Control, Physicians' Office Immunization Education, and Child Care Provider Education Classes.

 

Fees are established by the Board of Health, call for details.

Genetic Newborn Screening

Target Population:  Newborn

 

This is a mandatory screening of all newborns in accordance with Ohio Administrative Code 3701.45.  The Health District will provide this screening if there is a home birth with no physician or midwife in attendance or when a physician is unable to locate a baby who needs a second test or retest.

Health Assessments

Target Population:  Birth to 18 years of age

 

This is a service that Clinical Services provides to perform initial health screenings as needed for placement of foster/custody children through the Department of Job and Family Services.

Hepatitis C Antibody Testing

Target Population: Adults

 

This monthly clinic provides high risk individuals the opportunity to be screened for the Hepatitis C antibody at no charge.  Adults are seen by appointment.

Lead Screening

Target Population: Children 12 months to 6 years of age

 

Information on lead lesting, children at risk, and side effects of lead poisoning is offered routinely to clients in the Well Child Clinic and Lead Clinic.  Information on proper paint removal, water testing, etc. is given to clients upon request.  As appropriate, children with elevated leads are followed with additional screenings and/or referred to physicians and Health District Environmental Health according to Centers for Disease Control and Prevention standards.

 

All households with children with elevated lead levels receive a home visit by a public health nurse for in-depth screenings and education.  All children with elevated lead levels are referred to the Help Me Grow Program as appropriate. 

 

There is no fee involved for the testing.

Newborn Home Visit Program

Target Population: First time parents, teen-parents, and their newborn infants

 

Public Health Nurses make a one-time home visit in the first few weeks after the baby is born to provide an assessment of mother and child, answer questions, link families to services and resources within our community, and provide information to keep babies healthy and safe.  This newborn/maternal home visit is free of charge.

Rabies Vaccine

Target Population:  Residents of Lake County who had or may have a
potential exposure to Rabies

 

Communicable Disease Nurses are responsible for providing information on how to obtain Rabies vaccine for use in post-exposure therapy.  Technical information on Rabies disease and Rabies vaccine is provided to local physicians upon request.  An information packet about Rabies and
Rabies vaccine is available to clients/families in need of post-exposure vaccine.

Seasonal Influenza

Target Population:  Six months of age and older

 

Annual vaccination against influenza is recommended for anyone who wants to reduce the risk of becoming ill with influenza or transmitting it to others. 

 

Fee is reviewed annually and established by the Board of Health.

Tuberculosis Control Program

Target Population:  Community at large with focus on high-risk populations such as: the elderly, foreign-born, incarcerated, homeless, and other long-term care residents

 

 

Communicable Disease Nurses receive reports of suspect and confirmed cases of active tuberculosis (TB) for contact identification and follow-up.  Prevention efforts include medication therapy for positive reactors and cases and providing TB Testing Clinics and client education. 

 

Fees for TB Testing are reviewed annually and established by the Board of Health.

Well Child Clinic
Target Population
:  Underinsured children 0-21 years of age

This program provides comprehensive well child health and developmental screenings and referrals.  Screenings are age appropriate and completed by a team consisting of a physician, several nurses, a social worker, and a dietitian.  Sports and work physicals, pre-school and school physicals, and well baby checkups are completed.  The Well Child Clinic remains partially funded by the Children and Families Health Services Grant.

 

Fees are based on a sliding fee scale set by the Ohio Department of Health.

Administration

  • Prepare staff to respond to bioterrorism or disaster-related emergencies.

Air Pollution Control

Target Population:  General

 

  • Local air authority for Lake and Geauga counties.
  • Monitor outside air for ozone, carbon monoxide, sulphur dioxide, particulates, and radiation.
  • Issue open burning permits.
  • Inspect demolition/renovation sites potentially containing asbestos.
  • Inspect dry cleaning facilities for emissions.
  • Conduct industrial stack readings.
  • Report Daily Air Quality Index.
  • Investigate complaints concerning air pollution sources.

Food Protection

Target Population:  General

 

  • License, inspect, and investigate complaints of restaurants, grocery stores, festivals, and temporary food stands.
  • Investigate communicable disease and foodborne illness reports.
  • Provide education and train food handlers through Person in Charge, ServSafe, and on-site classes.
  • Assist homeowners with food safety questions for holiday food preparation and
    emergency food situations.

  • Inspect schools with emphasis on sanitation, lab safety, vocational classrooms, and playgrounds.
  • Inspect jails to assure compliance with state standards.
  • License and inspect food services in schools, daycares and nursing homes.

Nuisance Complaints

Target Population:  General

  • Investigate complaints regarding licensed program compliance.
  • Investigate and/or provide advice regarding housing conditions, mold,
    bedbugs, gas and oil well industry, pesticide safety, solid and hazardous
    waste disposal, rodents, animal waste, cancer clusters, water quality,
    underground storage tanks, occupational health, daycare, clandestine
    methamphetamine labs, and lead paint.
  • Refer clients to outside agencies as appropriate for assistance.

Plumbing

Target Population:  General

  • Issue permits and inspection of commercial plumbing in Lake County communities except Eastlake, Mentor and Painesville City.
  • Issue permits and inspect residential plumbing in all townships and villages in addition to Kirtland, Willowick, and Mentor-on-the-Lake cities.

Rabies Control

Target Population:  General

 

  • Investigate all animal bites and potential rabies exposures to assure the  victim is not at risk of rabies infection.
  • Enforce local rabies vaccination regulations.
  • Enforce confinement of domestic animals that have bitten a human.
  • Facilitate laboratory examination of wild and domestic animals that are suspect of
    being rabid and have exposed a human or domestic animal.
  • Assist state and federal agencies with distribution of raccoon rabies vaccine.
  • Obtain samples of rabies-suspect animals for surveillance.
  • Consult with residents who may have been exposed to a bat in their home.
  • Provide guidance to the medical community on rabies or other zoonotic disease.

Radiological Health

Target Population:  General

 

  • Conduct routine/emergency monitoring and sampling for the Perry Nuclear Power Plant.
  • Monitor ambient air for presence of radiation.
  • Distribute Potassium Iodide (KI) to residents and businesses.
  • Maintain a trained team to respond in the event of a radiological release at the Perry Nuclear Power Plant.

Sewage Treatment and Stormwater Management

Target Population:  General

 

  • Inspect home and semi-public sewage treatment systems.
  • Evaluate vacant lots to determine if sewage system installation is feasible.
  • Evaluate existing home sewage systems as requested prior to home sale.
  • Register system installers, septic tank pumpers, and service providers.
  • Issue orders to abandon septic tanks and connect to sanitary sewers.
  • Provide storm water management activities such as illicit discharge detection,
    elimination, prevention, and education in contracting communities.

Target Population:  General

  • Investigate complaints of alleged violations of smoking ban in public places or places of employment.
  • Educate public regarding elements of smoke free workplace program.

Solid Waste Disposal

Target Population:  General

 

  • License and inspect the Lake County Solid Waste Facility and closed landfills.
  • Monitor explosive gas (methane) at landfills.
  • Inspect infectious waste generators and treatment facilities, fly ash disposal, yard waste, regulated compost sites, scrap tire, and construction and demolition waste.
  • Register and inspect tattoo and body piercing establishments.
  • Sponsor drug disposal locations for unwanted and unused prescription and
    over-the-counter medications.

Vector Borne Illness

Target Population:  General

 

  • Investigate potential source of mosquito and tick-related illness.
  • Advise residents regarding disease prevention.
  • Conduct mosquito population surveillance.
  • Apply larval and adult mosquito control pesticides
  • Identify mosquitoes and ticks.
  • Advise public on at-home mosquito control.

Health Promotion and Planning

 

ACHIEVE Wellness Lake County

Target Population:   Business, community, and government agencies

 

 

 

 

 

 

ACHIEVE Wellness Lake County, which stands for Action Communities for Health, Innovation, and Environmental Change, is a county-wide partnership that consists of city and county health officials, city and county government, parks and recreation departments, the Lake County YMCA, and representatives from local businesses, community agencies, and schools.  This collaborative partnership helps to implement policy and environmental change in the areas of physical activity and nutrition.

 

  • The Community Health Action Response Team (CHART) is a group of key stakeholders in Lake County that come together every other month to assess the needs of the community and work on action plans that include targeted policy and environmental changes.
  • CHART makes up five sub-committees that meet every other month to address needs within the specific sector.

    • ACHIEVE Wellness Lake County provides technical assistance to agencies and schools when implementing physical activity and nutrition changes.

Action for Healthy Kids

Target Population:  School districts, parents, and community agencies

  • Provide technical assistance to Lake County schools to improve the
    health and academic performance of children through improved
    nutrition and physical activity in schools.
  • Member of the Action for Healthy Kids Zone 4 Coalition.
  • Involve students, educators, families, and community volunteers in a
    framework that supports school based wellness activities and long term
    policy change.
  • Provide educational opportunities for educators, professionals, school
     personnel, etc. who are interested in improving nutrition and physical activity
    in the schools.

B Fit 4 Life Corporate Challenge

Target Population:  Lake County agencies, businesses, and worksites

The B Fit 4 Life Corporate Challenge is a five month initiative ( January

through May) among local businesses to increase their level of physical activity and overall fitness.  Each month includes a main fitness event and alternative activities.  Points are accumulated based on the team's participation level.  The goal of each team is to obtain the maximum amount of participation from the entire team.  Teams can have a minimum of five to ten employees or a maximum of 20.  Each team member needs to complete 3 of the 4 monthly events; 1 of which is pedometer walking.  The idea behind B Fit 4 Life is to encourage employees to be active all year long.

Car Safety Seat and Booster Seat Program

Target Population:  Low income families and children

 

This program provides child safety seats and booster seats to low-income and economically disadvantaged families.  To be eligible for a reduced cost child safety seat or booster seat, the following requirements must be met:

  • Child is no less than 5 pounds.
  • Parent/guardian agrees to pay a cost of $30.00 for a convertible or a high back booster seat or $5.00 for a backless booster seat.
  • Parent/guardian has a vehicle with working safety belts for transportation of the child and brings this vehicle to the class.
  • Family is determined to be at 185% or less of the federal poverty income guidelines.
  • To receive a convertible or high back booster seat, the parent/guardian must attend a one hour educational class on the proper use and installation of a child safety seat.  Backless boosters are available on a walk-in basis.
  • Parent/guardian agrees to the proper use of the child safety seat each time the child rides in the vehicle.
  • Parent/guardian agrees to the proper use of their safety belts every time they ride in the vehicle.

Emergency Preparedness

Target Population:  General

A public health emergency is any event or situation needing immediate action from Lake County General Health District (LCGHD) to prevent disease and injury or maintain public health within Lake County.  Emergencies can be natural or man-made.  Examples of past Lake County public health emergencies include flooding, derailment of a train carrying hazardous materials, and H1N1 influenza. 

 

  • Public Information:  All news releases, fact sheets, and other informational material for LCGHD are generated through Emergency Preparedness. 
  • Public Health Emergency Planning:  Some LCGHD emergency response plans are available online for public comment at www.lcghd.org .
  • Training, Exercise Development, and Execution:  Emergency Preparedness develops training and exercises for LCGHD staff and Public Health Partners, including Medical Reserve Corps volunteers, to keep them current on plans for a successful public health response when needed.
  • Point of Dispensing (POD) Sites:  Emergency Preparedness collaborates with local public safety agencies to plan for medication distribution clinics during a public health emergency that requires that the public pick up life saving medication and/or vaccine.
  • Individual Preparedness Education:  Emergency Preparedness has educational materials to help the public, schools, businesses, and others prepare for an emergency.

The Lake County Healthy Lifestyle Initiative was created in response to the 2010 Lake County Community Health Assessment where adult obesity and childhood obesity were the 1st and 3rd concerns of Lake County residents.  As the primary agency for community health services, we understand it is our obligation to provide health and wellness interventions.  Through education, awareness, and practice, we work to engage the community in healthy lifestyles that can be maintained through all stages of life.

The Lake County Childhood Obesity Prevention Initiative provides school, community, and family based education programs with a focus on making healthy diet, lifestyle, and environmental changes.  The Healthy Families Resource Center offers school based support to children identified as having potential body weight concerns.  The center provides office based counseling to the family, not just the child.

HIV / AIDS Case Management

Target Population:  Individuals who have a diagnosis of HIV/ AIDS
Primary service areas are Lake, Ashtabula, and Geauga counties.

 

Program Goals:

To enhance the quality of life and well being for individuals living with AIDS and HIV

by providing connections to comprehensive services for:

  • Medical care
  • Mental health services
  • Drug and alcohol treatment
  • Housing opportunities
  • Medical nutrition therapy for qualifying clients

  • Medical case management visits can be facilitated in home, office, or community settings
  • Access point for utilization of Ryan White funds/assistance
  • Transportation assistance available
  • Housing Opportunities for People Living With AIDS (HOPWA) access point
  • Links with medication assistance programs (OHDAP).
  • Community referrals and resource support provided
  • Support Groups (facilitated by licensed social worker) are available
  • HIV education

Lake County Drug Assistance Program

Target Population:  All Lake and Geauga county residents

 

The Lake County Drug Assistance Program assists the uninsured or underinsured in

obtaining medications at a lower cost.  This assists individuals in medication compliance
for chronic diseases or illnesses.

Lake County Medical Reserve Corps

 


Lake County Medical Reserve Corps

Target Population:  Volunteer medical professionals

 

This program provides medical professionals with the opportunity to volunteer in the area of emergency preparedness and response.  Volunteers may work and/or live in Lake County and must hold a medical license/certification or work in a medical capacity.  The goal of this program is to better prepare our county in the event of an emergency.

 

  • New volunteers must meet with the Lake County Medical Reserve Corps (MRC) Coordinator to complete volunteer information forms and be properly introduced to the program.
  • New volunteers must register with the state of Ohio on the Ohio Responds database as a volunteer with the Lake County MRC unit.
  • Volunteers must complete four hours of approved training every three years.
  • A background check will be conducted on every volunteer.
  • An ID badge will be created for every volunteer.
  • Volunteers are asked to participate in quarterly Ohio Responds test activation alerts.  Volunteer MRC meetings are scheduled as needed and there will not be more than two per year.
  • Emergency exercises, drills, and other preparedness efforts are held at various times when the opportunity arises.

Lake Geauga Ashtabula Tobacco Prevention Coalition

Target Population:  General

 

This Coalition is a partnership of organizations and individuals with the common goal of promoting tobacco free living through tobacco education and prevention to the benefit of all people living in Lake, Geauga, and Ashtabula.

Traffic Safety Education - Lake County Safe Communities Coalition
Target Population:  All ages

 

  • The Safe Communities program focuses on lowering the incidence of traffic safety injuries in Lake County.  In cooperation with local police departments, surveillance and
     traffic safety education occurs throughout the year.
  • The Lake County Safe Communities Coalition is a community based approach to addressing motor vehicle crashes, injuries, and deaths.  The Coalition's focus is seat belt usage, impaired driving, and motorcycle safety.
  • The Coalition participates in national campaigns such as Drive Sober or Get Pulled Over and Click It or Ticket, as well as, other traffic safety initiatives throughout the year.

Healthy Families

 

Autism Connection of Lake County

Target Population:  Children birth to five years of age with suspected autism

 

Providing autism and developmental screenings, evaluations, and community resources, such as educational, medical, advocacy, referral, and support services. 

Engage families, child care providers, educators, health care professionals, and community leaders in identifying ways to promote early identification of autism using standardized methods, while facilitating regional coordination of linkages with timely, effective services for children with autism and other developmental delays.

Fatherhood Initiative

Target Popualtion:  Children birth to five years of age and fathers

Offering fathers of children under the age of five years home visitation and group activities to increase parent knowledge of early childhood development, health, safety, reduction of stressors, and improved parenting practices.  Support services include prenatal, bonding, and
paternity education and locating community resources unique to Lake County fathers.

Great Gains   Helping families get their children ready for kindergarten

Target Popualtion:  Birth to kindergarten

Great Gains is designed to assist high risk families with kindergarten readiness. The Great Gains home visitor helps the family reinforce skill building at home, navigate the system, and engage in services available to children with unique needs.

 

Key outcomes include:

Help Me Grow

Target Population:  Pregnancy to preschool

 

Research shows that during prenatal to five years of age there is a critical window of opportunity for rapid growth of the brain, language, motor, cognitive, adaptive, and social-emotional skill development that lasts a lifetime.  This window of opportunity is extremely important for children who require extra support to reach their full developmental potential and for children at risk of not meeting their developmental potential. 

 

Help Me Grow supports the well being of pregnant women and young children through home based specialized service, prenatal to three years of age parenting education, community resource linkages, and public awareness.  A special emphasis is placed on early prevention and intervention and support to first time/teen mothers to ensure that all young children in our community are offered the opportunity to reach their full developmental potential during this critical period in a child’s life.

 

Families are offered:

  • Full developmental screenings that include vision, hearing, and nutrition
  • Prenatal education for expecting parents
  • Home visitation services for case management and parenting education
  • Linking and referrals to community services
  • Full comprehensive developmental evaluations for children with medical diagnosis or suspected developmental delays

 

Early Intervention services are offered in partnership with the Lake County Department of Developmental Disabilities.

VITAL STATISTICS

Although Ohio records of births and deaths are filed within the registration district in which the event occurred, birth certificates are available in Lake County regardless of where the birth occurred.  No matter where someone is/was born in Ohio, the Health District Vital Statistics department can issue that birth certificate.  Currently, death certificates are filed and issued only for Lake County deaths.  You may stop in at the office of the Lake County General Health District and obtain a certified birth or death certificate for $21.50 per certificate.  Cash, checks, and credit cards (except for American Express) are accepted.

For genealogical purposes, computerized birth and death indexes may be searched between the hours of
10:00 a.m. and 2:00 p.m. Monday through Friday; no appointment is necessary.  If there are other people waiting to view the indexes, time on the computer will be limited to one-half hour per visit.  A place to work is provided in the Administration area where the guest computer with index files is located.

WIC (WOMEN, INFANTS and CHILDREN) PROGRAM

Target Population:  Lake County women who are pregnant, breastfeeding
 and/or postpartum, and children 0-5
years of age

SERVICES:  WIC provides individualized nutrition education and counseling and breastfeeding education and support.  WIC is a supplemental nutrition program for women, infants, and children up to the age of five.  Foods are individually prescribed from
registered and licensed dietitians to ensure adequate and appropriate nutrition.  WIC offers
nutritious foods to promote infant, early childhood, prenatal, and postpartum nutrition. 
Some other services offered include a peer helper program promoting breastfeeding and
support, and breastfeeding consultations with certified lactation counselors.  WIC acts as
a resource for several diverse community services including: Medicaid, Healthy Start,
Child and Family Health Services, BCMH, Well Care, and several others.

GOALS:        

  • To improve pregnancy outcomes
  • Reduce infant mortality rates
  • Reduce low birth weights
  • To provide healthy eating guidance and information to parents of infants and children to the age of five
  • To provide nutritious foods to income eligible and qualifying families
  • To provide breastfeeding information and help increase breastfeeding rates and promote healthy babies in Lake County
