All orders placed online are subject to a $5.00 processing fee.
Lake County General Health usually processes requests on the same business day the order is submitted. Effective 2/5/2013 Health District mail is sent out at 10:00 a.m. each day (one mailing per day), which means that your order will be mailed on the next business day. Standard delivery method is U.S. Mail. Overnight delivery is available by FedEx for an additional shipping charge which varies. ($20.00 - $30.00+ currently - depending on destination). If you want to expedite the shipping process for faster delivery than U.S. Mail, please call (440) 350-2549 or (440) 350-2455 to receive a quote on the FedEx cost. You may stop in at the office of the Lake County General Health District (Directions) and obtain a certified birth certificate at the current price of $21.50 per certificate (same day service).
You may download the form below, fill out the required information and then mail or bring in the application with the appropriate fees to the Lake County General Health District.
