Birth Records Ohio records of births are filed within the registration district where the event occurred. If the birth occurred within Lake County, the record is on file at the Lake County General Health District. If the birth occurred outside of Lake County, but within Ohio, the certificate can be obtained from the Ohio Department of Health. Ways To Obtain Birth Records from Lake County You may stop in at the office of the Lake County General Health District (Directions) and obtain a certified birth certificate at the current price of $21.50 per certificate.  You may download the Application For Certified Copies Form , complete the information and then mail or bring in the application with the appropriate fees to the Lake County General Health District. Please contact the Deputy Registrar by telephone at (440) 350-2549, or by E-mail, JBryan@lcghd.org , for additional information on obtaining a birth certificate. Following is a list of our fees: Certified Birth Certificate $21.50 Birth List (per month)   $5.00
