| Ohio records of deaths are filed within the registration district where the event occurred. If the death occurred within Lake County, the record is on file at the Lake County General Health District. If the death occurred outside of Lake County, but within Ohio, the certificate can be obtained from the Ohio Department of Health.
Ways To Obtain Death Records from Lake County
You may come into the office and purchase a certified death certificate for $21.50.
You may download the Application For Certified Copies Form , complete the information and then mail or bring in the application with the appropriate fees to the Lake County General Health District.
