Eligibility Requirements for the Drug Assistance Program
Must be a resident of Lake County, Ohio,
Must be able to show a Driver's license or State ID
People that are 65 and under without insurance:
- You must be at or below the 250% poverty level
People 65 and over with insurance:
People that are 65 and under and have prescription coverage:
- If your prescription is not covered under your insurance plan, the cost of the prescription must be over $100.00 for a one month supply to qualify
People with Medicaid are eligible
- Although they need to have Medicaid only, they can't have Medicaid and Medicare benefits
