A communicable disease can be reported in Lake County in one of the following ways:

Ask for the communicable disease nurse by calling 440-350-2554.
Fax to a confidential fax line at, 440-350-2956.
Phone and ask for communicable disease confidential voice mail; if a communicable disease nurse in unavailable.

To immediately report an urgent communicable disease, an outbreak or a public health emergency, call 440-350-2554 Monday through Friday 8:00 am to 4:30 pm. After hours call 440-974-5009.
