Does Your Child Have Asthma?
If your child has asthma and is between the ages of 4-18, has been previously diagnosed with asthma by a physician, and lives in Lake County, then you qualify for the Breathe Well, Breathe Free program.
Families that agree to participate in the program will receive home-based education from a Health Educator, Registered Nurse, and a Registered Dietitian that focuses on asthma, nutrition, and physical activity. The program will identify asthma triggers in the home and provide ways to reduce the triggers. Educational materials will be given at each session. There are nine sessions.
